DPH has developed a comprehensive online application to help patients enroll in public health programs, assess certain conditions, and track treatment progress. Central to the project is using eForms for patient health assessments. DPH found that a commercial eForms application could provide the required flexibility and reliability at a lower cost.

DPH assessments are conducted in the field on wireless laptops and at clinics by public health workers. The assessments require flexible validation and scoring to ensure the quality and consistency of the gathered data. Proven assessments are housed in a question library.

DPH chose Cogix because it allows health professionals to create a customizable library of validation and scoring measurement instruments without requiring further system integration, for its role-based security, and for its ability to integrate and exchange data with the overall system.
